How to complete the application

If you are a Benefit Recipient (the person entitled to Federal benefits)

Complete Sections 1 and 4 on the Direct Express Application

Mail in the completed and signed application, along with any documents from Section 3 of the application, that are relevant to the address below.

If you are a representative Payee (someone who receives payment on behalf of a benefit recipient)

When the application is received

When your documents and application for the Direct Express is received, your personal information will be verified and the federal agency will confirm your payment. You should receive your Direct Express debit card in 7 to 14 days. Your payments will then be automatically deposited to your new debit card account on the same day you currently receive your regular paper check. You may receive one more paper check before your payments are switched to the new Direct Express debit card.

Applying for a Direct Express Debit Card is easy. There is no credit check or minimum balance requirement. If you are receiving Social Security or Supplemental Security Income benefits you can call Direct Express card enrollment at 1 (800) 333-1795 or find the application to print at USDirectExpress.com.

  1. I accidently changed my Social Security payments from DirectExpress to my former bank and they had already closed my account. They sent my check back to Social Security. Could you please change my direct deposit back to DirectExpress? I still have my card. Please Helen Diehl
